Buy cheap Generic Topamax (Topiramate) without dr prescription at Canadian Pharmacy

At our US pharmacy, you can purchase Topamax (topiramate) 25 mg, 50 mg, 100 mg, and 200 mg tablets without a prescription, with discreet and anonymous packaging delivered within 5-14 days across USA.
Topamax is an anticonvulsant used to treat epilepsy in adults and children, and to prevent migraine headaches in adults. It also has a place in the treatment of Lennox-Gastaut syndrome, a severe form of childhood epilepsy. The medication works by calming overactive electrical signals in the brain.
Dosing starts low and goes up slowly. A common starting dose is 25 mg once daily at night for one week. After that, the dose is raised by 25 mg to 50 mg per day at one- or two-week intervals, depending on how you respond and how well you tolerate it. For epilepsy, the usual maintenance range is 100 mg to 200 mg per day, divided into two doses. For migraine prevention, the target is usually 100 mg per day, also in two divided doses.
The administration form is film-coated tablets in 25 mg, 50 mg, 100 mg, and 200 mg strengths. Sprinkle capsules are also available in some markets for people who have trouble swallowing tablets.
The medication does not work overnight for migraines. It may take several weeks of steady dosing before you notice fewer headaches. For epilepsy, the full effect on seizure control can take even longer.
Alcohol should be avoided during treatment. It adds to the drowsiness and dizziness that topiramate can cause, and it can also increase the risk of metabolic acidosis and kidney stones.
The most common side effects are tingling in the hands and feet, drowsiness, dizziness, and weight loss.

Common Use Topamax

Topiramate is a sulfamate-substituted monosaccharide. Its exact mechanism is not fully understood, but researchers have identified several actions that likely work together. It blocks voltage-dependent sodium channels, which dampens the repetitive firing of neurons that can lead to seizures. It also enhances the activity of GABA, the brain's main inhibitory neurotransmitter, at certain GABA-A receptor subtypes. It antagonizes AMPA and kainate receptors, which are involved in excitatory glutamate signalling. And it inhibits some carbonic anhydrase enzymes, though this effect is weaker than that of acetazolamide and probably plays a minor role in its antiepileptic action. Health Canada has approved topiramate as monotherapy for partial-onset seizures and primary generalized tonic-clonic seizures in adults and children over 6 years of age. It is also approved as adjunctive therapy for partial-onset seizures, primary generalized tonic-clonic seizures, and seizures associated with Lennox-Gastaut syndrome in patients aged 2 years and older. For migraine, it is indicated for prophylaxis in adults after careful evaluation of other treatment options. It is not meant to treat an acute migraine attack once it has started.

Dosage & Direction

Topamax is always started at a low dose and increased gradually. The usual starting dose for adults is 25 mg once daily, taken at night to minimize daytime drowsiness. After one week, the dose is increased by 25 mg to 50 mg per day, with the increments spaced at one- or two-week intervals. For epilepsy monotherapy, the recommended initial target is 100 mg to 200 mg per day in two divided doses. The maximum recommended dose is 500 mg per day, though some patients with refractory epilepsy have tolerated up to 1,000 mg per day under close supervision. For adjunctive epilepsy therapy, the starting dose is 25 mg to 50 mg per day, and the usual maintenance range is 200 mg to 400 mg per day in two divided doses, with a maximum of 800 mg per day. For migraine prevention, the starting dose is 25 mg nightly for one week, then increased by 25 mg per day at weekly intervals as needed. The recommended total daily dose is 100 mg, given in two divided doses. Some patients do well on 50 mg daily, and a few may need up to 200 mg daily. Take the tablets with or without food. Swallow them whole. Do not crush or chew them, because they have a bitter taste. If you have kidney problems, your doctor will likely start you on a lower dose and titrate more slowly, because topiramate is cleared by the kidneys. If you are on dialysis, the dose needs to be adjusted around your dialysis schedule. Do not stop topiramate suddenly. Withdrawal should be gradual to avoid rebound seizures or migraines. In clinical trials, the dose was reduced by 50 mg to 100 mg per week in adults with epilepsy, and by 25 mg to 50 mg per week in adults taking up to 100 mg daily for migraine prevention.

Precautions

Topiramate can cause metabolic acidosis, a condition where the blood becomes too acidic. This happens because topiramate inhibits carbonic anhydrase in the kidneys, which reduces bicarbonate reabsorption. The drop in serum bicarbonate is usually mild to moderate, averaging about 4 mmol/L at doses of 100 mg per day or higher in adults. In rare cases, bicarbonate can fall below 10 mmol/L. Chronic, untreated metabolic acidosis increases the risk of kidney stones and nephrocalcinosis, and it may lead to osteopenia or osteoporosis. In children, chronic metabolic acidosis can slow growth. Conditions that predispose to acidosis, such as kidney disease, severe respiratory disease, status epilepticus, diarrhea, surgery, or a ketogenic diet, can worsen topiramate's effect on bicarbonate. If you develop symptoms like rapid breathing, fatigue, loss of appetite, nausea, vomiting, or irregular heartbeat, contact your doctor. Topiramate can cause kidney stones. The risk is higher if you have had stones before, have a family history of stones, or have high calcium in your urine. Drink plenty of fluids throughout the day to help prevent stones. Topiramate can cause acute myopia and secondary angle closure glaucoma, a serious eye condition that can lead to permanent vision loss if not treated. Symptoms include sudden blurry vision, eye pain, redness, and seeing halos around lights. Stop the medication and seek emergency care if these occur. Cognitive side effects are common with topiramate. People report problems with attention, concentration, memory, and word-finding. These effects are dose-related and often improve with time or a lower dose, but some people find them persistent. Do not drive or operate machinery until you know how topiramate affects you. Topiramate can cause decreased sweating and increased body temperature, especially in children. This is more likely in hot weather or during physical activity. Watch for signs of overheating, such as fever, lack of sweating, and lethargy. Topiramate can cause hyperammonemia, with or without encephalopathy. This is a buildup of ammonia in the blood that can cause confusion, lethargy, and vomiting. The risk is higher if you also take valproic acid. Tell your doctor if you take valproate. Antiepileptic drugs, including topiramate, increase the risk of suicidal thoughts and behaviour. This risk applies to everyone taking the medication, not just those with epilepsy. Watch for new or worsening depression, anxiety, or thoughts of self-harm, and report them to your doctor right away. Topiramate is not recommended during pregnancy. It has been associated with an increased risk of oral clefts in babies exposed during the first trimester. Women who could become pregnant should use effective contraception and discuss the risks with their doctor. Topiramate passes into breast milk, so breastfeeding is not recommended without medical advice. If you have liver or kidney disease, your doctor should monitor you closely. Topiramate can also worsen liver function in people with pre-existing liver problems.

Contraindications

Do not take Topamax if you have had an allergic reaction to topiramate or any of the inactive ingredients in the tablet or capsule. The medication is contraindicated in patients with a history of metabolic acidosis, because it can worsen the condition. Topiramate should not be used during pregnancy unless the potential benefit clearly outweighs the risk. It is contraindicated in women who are breastfeeding, because it passes into breast milk and can cause serious adverse reactions in the infant. Do not take topiramate if you have severe kidney impairment without close medical supervision and dose adjustment. The medication is not recommended for the treatment of acute migraine attacks. It should not be used in patients with a history of kidney stones without careful evaluation of the risks and benefits. Topiramate is contraindicated in patients with glaucoma, specifically angle-closure glaucoma, because it can trigger an acute attack. If you have any condition that predisposes you to metabolic acidosis, tell your doctor before starting topiramate.

Possible Side Effects

More than 1 in 10 people experience tingling or numbness in the hands and feet, drowsiness, dizziness, fatigue, and weight loss. These are the most common effects and are usually dose-related. Other common side effects include nausea, diarrhea, loss of appetite, changes in taste, and difficulty with memory or concentration. Some people notice mood changes, including depression, irritability, or anxiety. These effects often improve as your body adjusts, but if they persist or bother you, tell your doctor. Serious side effects require immediate medical attention. Metabolic acidosis can cause rapid breathing, fatigue, nausea, and irregular heartbeat. Acute angle-closure glaucoma presents with sudden eye pain, blurred vision, and redness. Kidney stones cause severe pain in the side or back, blood in the urine, and painful urination. Hyperammonemia with encephalopathy can cause confusion, lethargy, and vomiting. Suicidal thoughts or behaviour can emerge, especially in young people. Oligohydrosis and hyperthermia cause decreased sweating and overheating, particularly in children. Severe skin reactions, including Stevens-Johnson syndrome and toxic epidermal necrolysis, have been reported rarely. If you develop a rash, blisters, peeling skin, or sores in the mouth or eyes, stop the medication and seek emergency care. Cognitive effects, including problems with word-finding and memory, are common and can be distressing. If they interfere with your daily life, talk to your doctor about adjusting the dose. Most side effects are manageable with dose adjustment or time, but any symptom that concerns you should be reported to your doctor.

Drug Interaction

Topiramate interacts with several medications. Carbonic anhydrase inhibitors, such as acetazolamide, dichlorphenamide, and zonisamide, add to the risk of metabolic acidosis and kidney stones. Avoid combining these unless your doctor specifically directs it. Valproic acid, used for epilepsy and bipolar disorder, increases the risk of hyperammonemia and encephalopathy when taken with topiramate. Your doctor should monitor you for symptoms if you take both. Topiramate can reduce the effectiveness of oral contraceptives at doses of 200 mg per day or higher. Use a backup method of contraception while taking topiramate. It can also increase the clearance of lithium, reducing its levels and effectiveness. Your doctor should monitor lithium levels if you take both. Topiramate may increase the effects of central nervous system depressants, including alcohol, benzodiazepines, and opioids, leading to excessive sedation. It can also affect the levels of other antiepileptic drugs. When topiramate is added to phenytoin, phenytoin levels may need adjustment. When carbamazepine or phenytoin is added to topiramate, topiramate levels may decrease and the dose may need to be increased. Metformin and pioglitazone levels may be affected. Ketogenic diets add to the risk of metabolic acidosis. Topiramate inhibits the enzyme CYP2C19 and can interfere with other substances metabolized by this enzyme, including diazepam, imipramine, moclobemide, proguanil, and omeprazole. St. John's wort may reduce topiramate plasma concentrations, leading to loss of efficacy. Tell your doctor and pharmacist about every medication you take, including over-the-counter products, vitamins, and herbal supplements.

Missed Dose

If you miss a dose of Topamax, take it as soon as you remember. If it is almost time for your next dose, skip the missed one and continue with your regular schedule. Do not take two doses at once to make up for a missed one. If you miss several doses in a row, contact your doctor before restarting. Your body may need time to readjust, and your doctor may want you to start at a lower dose again. Missing doses can increase the risk of seizures or migraines, so try to take the medication at the same times each day.

Overdose

Overdose with Topamax can cause severe drowsiness, confusion, agitation, hallucinations, seizures, and coma. Other symptoms may include double vision, speech disturbances, dizziness, low blood pressure, and irregular heartbeat. There is no specific antidote. Treatment is supportive and may include activated charcoal if the overdose was recent, intravenous fluids, and monitoring of heart rhythm and vital signs. Topiramate is not effectively removed by hemodialysis, so dialysis is not a useful treatment for overdose. Contact a poison control centre or go to an emergency department immediately if you or someone else has taken too much. Do not try to manage an overdose at home.

Storage

Store Topamax tablets and sprinkle capsules at room temperature between 15 and 30 degrees Celsius (59 and 86 degrees Fahrenheit). Keep the container tightly closed and protect it from light and moisture. Do not store in the bathroom or near a sink. For sprinkle capsules, you can swallow them whole or open them and sprinkle the contents on a teaspoon of soft food. Eat the mixture immediately. Do not chew it and do not store it for later use. Keep all forms of topiramate out of reach of children and pets. Do not use after the expiry date on the label. Dispose of unused medication according to local guidelines. Do not flush topiramate down the toilet or pour it down the sink.
